HOW TO REQUEST SURVEILLANCE DATA
Departments from the Local Government of Quezon City, Researchers from different organizations,
agencies, and academe, and other individuals who want to request surveillance data therefor can request
data from Quezon City Health Department through the Quezon City Epidemiology and Surveillance
Division.
(Quezon City Epidemiology and Surveillance Division ensures adherence to the guidelines set forth in
the Republic Act 10173, also known as the Data Privacy Act of 2012 for every requested data)
