RESERVATION PROCESS FOR FREE SWAB TEST
The program provides free COVID-19 Tests to all Quezon City residents and Quezon City workers included in the Priority Groups. Free COVID-19 Testing is conducted at various Quezon City Health Centers.
Priority group
- Close Contact with a Positive Individual whose last exposure is within seven (7) days
- With ongoing Symptoms
- Pregnant woman and companion
- Dialysis and Chemo Therapy Patient
- Individuals that will undergo Medical Operation
- Fill out this Reservation Form: (http://bit.ly/QCfreetest).
- Choose the reason for swab.
- Wait for the Verification Call from our CBT Verifier.
- If your request has been accepted, wait for the confirmation text that includes the schedule of your swab, place of swab, and unique code to be presented on the swabbing site.
- During the scheduled date of the swab, please present a valid Identification Card, preferably a PhilHealth ID, and Vaccination Card along with the Confirmation Text given by the QCESD CBT Verifier.
- Travel Purposes, On Job Training (OJT) or Internship, and other non-COVID-19 reasons are not a priority in QC Free Swab testing.
- If there is a COVID-19 transmission in the workplace, please coordinate immediately with QCESD via COVID-19 Hotline
- QCESD does not allow re-swab testing, especially if non-medical purposes. Assessment from the QCESD Medical Field Team will be conducted.
- Patient/s that will avail of Free COVID-19 Swab test for Back to Work or Pre-Employment Requirement must provide a Request Letter signed by his/her employer.
- Patient/s that is required to undergo Swab testing for Medical Purposes like surgery, check-up, and dental purposes should provide a Doctor’s Request for Swab or Referral.
